It is our goal to make your Physical Therapy visits and your insurance billing go as smoothly as possible.  To aide us in this goal, please bring the following items to your first visit.

1. All insurance cards for companies you wish us to bill.

2. A prescription from your doctor (or please alert us over the phone that you do not have one). *

3. A Pre-authorization form if it is required by your insurance company.

4. A form of photo identification.

 *If you do not have a prescription for Physical Therapy but believe you would benefit from treatment, we invite you to contact us to schedule an evaluation.  We will then coordinate your care with the doctor of your choice.

 Policies regarding billing and fees:

We participate with many insurance companies for submission and payment of your medical expenses.  Co-payments and co-insurance amounts are expected at the time of your appointment.  We accept  cash, and checks, as well as MasterCard, Visa and Discover. 

On your first visit, you will receive information about your insurance benefits.  Should you have any questions regarding your coverage, please ask to speak with our Patient Accounts Specialist.
